Twenty rows. In each row, pick the one word or phrase that sounds most like you on the job - not how you wish you were, and not how you are at home. Go with your first instinct; it is usually the accurate one.
Read this before the reflection questions - two of them ask what does and does not fit. Nothing here is fixed, and disagreeing with it is useful information, not a wrong answer.
Answer in your own words. These go straight into your report and are often the most useful part for your supervisor. Short answers are fine, and you may leave any question blank.
Scroll down to read it. Use Save as PDF to produce the file for the employee record - in the print dialog choose “Save as PDF”, Letter size, portrait, and turn on background graphics.
The .json file lets this report be reopened and re-printed later. Keep it with the PDF if you want an editable copy on file.
